Job Description:
Caring Companions In-Home Senior Care is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing support and oversight for clients receiving home care services. This position focuses primarily on conducting Medicaid nurse visits, performing health assessments, and ensuring each client’s care plan meets state requirements and promotes safety, comfort, and quality of life at home.
The ideal candidate is compassionate, detail-oriented, and comfortable working independently while maintaining excellent communication with caregivers, clients, and the administrative team.
Responsibilities:
Conduct Medicaid nurse visits and complete required health assessments and documentation
Review and update client care plans according to state guidelines
Monitor vital signs, medication compliance, and general health status
Provide education and guidance to caregivers and family members
Communicate changes in condition or care needs with the care coordination team
Maintain accurate and timely visit notes and reports for compliance
Ensure all services meet state and agency standards for quality and safety
Pay Rate:
$22-30 per hour, paid weekly
Requirements:
Active LPN license in the state of Missouri
Previous experience in home care, long-term care, or community health preferred
Strong assessment, documentation, and communication skills
Compassionate and professional bedside manner
Reliable transportation and valid driver’s license
Kind, dependable, and compassionate
Must pass background check and meet state credentialing requirements
Schedule:
Visits are typically 1 hour or less each
